These instructions are for current members annual renewal.  For new memberships, please go  here.

Annual renewals are due not later than December 31 of each year.   Dues are $100.00 per year but if the renewal is received after December 31, a penalty of $20.00 is imposed.

A full open AMA membership for the renewal year is required.  If the  AMA Membership Verification page shows a current membership, no further proof is required.  Otherwise, include a copy of the card or other verification.  If you include the actual card, the membership sticker will be applied and the card will be laminated and returned.

Complete the application at the keyboard, print it.
Sign the application, include check or money order and AMA proof (if required).
Mail to the address at the bottom of the application.
